House Account Form
By signing this authorization form, you acknowledge that you agree to authorize Durant’s to charge your credit card for all food, beverage, and service gratuities without your card being presented and without a receipt signed at the time of purchase. This form is your authorization and acceptance of all charges.
This authorization will remain in effect until it is canceled in writing, and notification is made in writing at houseaccounts@primesteakconcepts.com of any changes in account information or termination of this authorization 48 hrs before the next time the account is used.
In the case of a Transaction being rejected for any reason, Durant’s may at its discretion attempt to process the charge again within 30 days.
You may cancel anytime by contacting us directly via email at houseaccounts@primesteakconcepts.com. All charges will be billed directly to the contact person’s credit card on the same or next business day.
